Who must publish a modern slavery statement?

Under Section 54 of the Modern Slavery Act 2015, any commercial organisation supplying goods or services with a total annual turnover of £36 million or more, and carrying on business in the UK, must publish an annual slavery and human-trafficking statement. What must the statement contain?
